Sample requests must be in writing. You can submit requests via our online form by Clicking Here or you can fax orders to 440-632-1581, attn Special Markets Customer Service. If by fax, please include item number, description, quantity and your purchase cost.
Sample cost is the Net Brand or Blank distributor cost.
Terms: Credit Card and freight account with order and all special shipping instructions.
